Kaiser Permanente Provider Enrollment & Credentialing
Kaiser Permanente is a unique integrated health system that operates its own hospitals, clinics, and physician groups alongside its health plan. As such, Kaiser credentialing is unlike any other payer enrollment process — Kaiser typically employs its own physicians (through the Permanente Medical Groups) rather than contracting with independent providers. This means the traditional "credentialing" pathway for independent providers is extremely limited at Kaiser.
In most Kaiser regions, independent providers cannot become Kaiser-contracted providers at all. Kaiser's integrated model is designed to keep care entirely within its system. The exception is Kaiser's "out-of-area" or "supplemental" networks in some regions, where Kaiser contracts with independent providers when members travel or need services not available at Kaiser facilities.
For independent providers, the more likely interaction with Kaiser is through CAQH-independent credentialing when Kaiser members seek out-of-network care, or through Kaiser's specialty referral networks in markets where Kaiser lacks certain specialists internally. Arctic Health advises providers in Kaiser-dominant markets (Northern California, the Pacific Northwest, Colorado) on how to structure their payer mix to account for Kaiser's limited independent provider network.
CAQH is not required by Kaiser for its internal credentialing processes, as Kaiser uses its own internal credentialing database for employed and affiliated physicians. The timeline listed reflects the rare instances where Kaiser does credential independent providers, and it is substantially longer than typical commercial payer timelines.

If a network opportunity exists, complete Kaiser's internal credentialing application and attestation forms.
Submit required documents including license, DEA, board certification, and malpractice coverage.
Kaiser conducts an extensive primary source verification and peer review process.
Receive a formal credentialing decision from the regional Credentials Committee; effective dates are set quarterly in many regions.

Frequently Asked Questions
Can independent providers enroll with Kaiser Permanente?
In most Kaiser markets, independent providers cannot enroll in Kaiser's network because Kaiser employs its own physicians. Limited exceptions exist for out-of-area supplemental networks and specialty services Kaiser doesn't offer internally. Contact Kaiser's regional Provider Relations for current network opportunities.
What markets does Kaiser Permanente operate in?
Kaiser operates health plans in California (Northern and Southern), Colorado, Georgia, Hawaii, Maryland, Oregon, Virginia, Washington state, and the District of Columbia. Market presence varies significantly — Kaiser is dominant in Northern California but has a smaller footprint in Georgia and Colorado.
Does Kaiser Permanente use CAQH for credentialing?
No. Kaiser uses its own internal credentialing database and does not participate in CAQH ProView for credentialing employed or affiliated physicians. For the rare instances where Kaiser credentials independent providers, they use a proprietary internal application.
